Medicare Referrals

If a patient suffers from a condition that has lasted, or is predicted to last for at least 6 months, they may be eligible for medicare assistance under the Chronic Disease Management (CDM) Scheme.

An individual who has a Chronic Disease Management Plan can receive up to 5 Medicare rebatable exercise physiology sessions per calendar year.
